I purchased this less than a year ago from another user on Audiophile Style.  Since then I've had it in my rig less than 6 months.

 

In very good condition.

 

Setup in 12V mode and includes external master clock input and an additional clock tap.  The clock tap connects to the Netgear switch.  And to get you up and running right away, I've included an inexpensive power supply for the switch.
